Primary Yellow Line Project Goals and Measure

  1. Goal: Improved Access to Services

    Measures:
    1. Total # of screenings offered and total # screenings completed
    2. Actual # of individuals screened as eligible and referred to YLP
    3. Number of individuals screened provided information and referral (I&R) materials
    4. Type of I & R (Information and Referral) materials screened individuals requested and/or received

  2. Goal: Increased Engagement for Timely Services

    Measures:
    1. Staff Engagement: # of Law Enforcement referrals (track trends quarterly and annually)
      • Screening source and # referrals to YLP:
        • Blue Earth County Sheriff's Office
        • Mankato Department of Public Safety
        • Minnesota State Patrol
        • Other
    2. Recipient Engagement Rate: # of eligible recipients with a completed and signed My Yellow Line Plan - # eligible recipients referred to the Yellow Line Project (numerator/denominator)
    3. Timely Engagement Measure: Average (mean) from date of initial referral to date My Yellow Line Plan completed with signatures (track “mean” and “outlier range of short/long variance” - monthly, quarterly, and annually)

  3. Goal: Reduce Jail Bed Days for Divertible Individuals with Mental Illness and/or Chemical Dependency Needs that are Divertible

    Measures:
    1. Impact Measure: # of individuals with risk of higher incarceration days - offered and engaged in a plan for services:
      • # of street level referrals with a completed and signed My Yellow Line Plan
      • # of alternative placement diversions & PPR (prints, photos and release) referrals with a completed and signed My Yellow Line Plan
      • # short-term jail stay referrals with a completed and signed My Yellow Line Plan
    2. Success Rate: Individuals with a My Yellow Line Plan that successfully accomplished all identified goals on the plan within 60 days (track monthly, quarterly, and annually):
      • Successful completion
      • Unsuccessful completion
    3. New Contact Measure: Individuals who successfully accomplished a My Yellow Line Plan and consequently had no newly identified contact with Law Enforcement within three months, six months, and one year after My Yellow Line Plan case closure.
